It’s one of the most common pieces of advice people hear after an injury. And while strengthening absolutely has its place, there is something important that often gets missed.
You cannot strengthen tissue that has not finished healing.
At ALIVE Wellness Center, I see this all the time. Patients come in frustrated because they have been doing their exercises consistently, stretching, showing up to physical therapy, maybe even working with a trainer, and yet the pain is still there. Or worse, it flares every time they try to progress.
The issue usually is not effort.
It is timing.
What Happens When Tissue Is Still Injured
When a tendon, ligament, or muscle has been injured, the body goes through stages of healing. Inflammation, repair, and remodeling all need to happen in the right order.
If inflammation lingers or scar tissue forms poorly, the tissue never fully restores its strength or elasticity. That area may feel:
- Weak
- Tight
- Guarded
- Easily irritated
- Fine at rest but painful under load
When you layer strengthening exercises on top of tissue that is still inflamed or poorly healed, the body often responds with more protection. That protection shows up as tightness, compensation, or recurring pain.
It is not that strengthening is wrong. It is that the foundation is not ready.
Why Strength Alone Is Not Enough
Strengthening works best when the tissue can tolerate load.
If the underlying tissue is still:
- Inflamed
- Fibrotic
- Restricted by scar tissue
- Lacking proper circulation
then adding load can reinforce the problem instead of resolving it.
This is why some people feel stuck in a cycle of “better, then worse” every time they try to progress.
